Remove "Custom..." Security option when no custom friends group is set

Short, concise description of the idea
There is a usability problem here. (1) There is no reason for "Custom..." to appear in the Security menu, if the user has no custom groups defined. (2) Once an entry is posted, there is no way for the owner of the entry to differentiate between "Friends" and "Custom" security levels, since both are signified by the lock icon (with popup hint "[Protected]").

See above. Also see http://www.livejournal.com/support/see_request.bml?id=261717 for the confusion caused by the current user interface.
